Everything else is ultimately merely details -- important details, but variations on this theme. index.php is the control, the menu program determins what the "current page" is, and tons of occasions get fired in the process of construction that page. Plugin modules can hook in to those events and shift the work flow/furnish added information/etc. That is also a part of the reason so many Drupal resources focus on making modules. Without modules, Drupal does maybe not actually DO something other than say, 'Someone asked to get a page! Does this exist? No? OK, I'll serve up a 404.'
